Pei is a 32-year-old G1P0 woman who was been labouring at home but has now presented to the birthing unit at 39+1 week’s gestation with her partner Hans. Findings on abdominal palpation are: term fundus, longitudinal lie, cephalic presentation, and LOL position. Fetal movements are present and the fetal heart rate is 138bpm. Pei’s observations are all within normal limits and she is now contracting 3 in 10, with mild to moderate tone. Pei’s membranes are intact. Pei describes being well during her pregnancy but states “we are extremely anxious and don’t know what to expect in labour but are open to everything”. With Pei’s consent you perform a vaginal examination and the findings indicate that the cervix is 3cm dilated and fully effaced, the presenting part is at -1 and bulging forewaters are felt. Pei and Hans did not attend antenatal education classes and are now asking about options for pain relief in labour.
