Hi! So in my paper I chose the Planetree Model. The whole idea is to write a paper about the healthcare system practice model that I chose.

Systems
Based Practice is defined by the Department of Health & Human Services as the
provision of assessable, continuous, and coordinated person-centered care for
persons….through a system that incorporates a team approach, health information
technology, and shared decision making (www.hhs.gov).
This assignment gives you the
opportunity to select from the following and write a 3-4 page (750-1000 word) paper
about the healthcare system practice model that meets the stated objectives and
criteria in the grading rubric.

Defining Criteria:
Organization & Application of Systems
Based Practice Knowledge: The paper presents information in logical, meaningful,
and understandable sequence, and is clearly relevant to the topic and objectives.
The student must accurately and fully discuss
the topic in addition to providing examples of how the system impacts the
quality of care, client safety, and the role or potential role of the
professional nurse in the model.
APA format, grammar, spelling, and/or punctuation are
accurate, or with 0–minimal errors.
References/Resources Criteria: Support from the literature is present and includes a
minimum of three scholarly in-text citation and matching references.
