Excluding COVID-19, Identify a communicable or infectious disease that meets the following criteria:
Affects the state where you practice nursing
Is mandated to be reported to the state health department (A list of notifiable diseases should be available from your state health department’s website.)
That you are able to obtain recent data for (within the past 3 years) You need at least 4 references.

At a minimum, your writing should address the three content categories and subcomponents: 1) Disease & Causality ; 2) Risk & Rates of Occurrence; and 3) Control & Prevention. Each category will include background descriptions, epidemiological data, and relevant analysis demonstrating the significance of the findings and implications for disease control and prevention.

Transmission (host, agent, mode of transmission, environment)
Brief pathophysiology and potential complications
Relationship between exposure and transmission
Risk & Rates of Occurrence
Susceptibility and risk/protective factors
Populations at risk
Provide current epidemiological data for the disease within your state including but not limited to morbidity and mortality
Morbidity (incidence, prevalence, disability, expenditures)
Mortality (case fatality rate and/or survival time)
Trend analysis (changes over time, across regions, etc.)
Prevention & Control
Procedures for reporting and surveillance
Interventions aligned with prevention levels (see table below)
Implement at least one mode of intervention to decrease the risk of transmission.

This worksheet will assist you to develop your formal written paper. See the epidemiologic triad model (page 1-52 to 1-55 in Principles of Epidemiology) for further descriptions. You may choose to include the table in your submission with your written paper. Regardless of your approach, be sure to include your data source citations.
