Beginning and opening paragraph
Tell the reader the vulnerable or special population that you are going to discuss. Then you will discuss the factors leading to vulnerability, the significance and economic impact of the population. Further you will discuss the demographic and epidemiological data related to your population. Moving forward you will identify 2 health concerns using priority and then discuss implementation using the levels of prevention-primary, secondary, and tertiary.
Paragraphs
You will need to develop separate paragraphs for the 6 content areas in your discussion. . Look closely at the 6 content areas and discuss thoroughly. Several of the content areas have several items to discuss so you might consider the Level 1 and Level 2 headings. This is help you organize your discussion and help you know that you discussed everything. Remember to use separate paragraphs and develop the paragraphs thoroughly. Be sure that you cite each of your references in the body (discussion) of your project.
Remember to use terms that you have learned in this course. Using host, agent, and environment would be a great approach to writing your discussion. Also, using virulence, prevalence, incidence, and risk factors demonstrate that you have learned the concepts and can now apply the concepts in a paper. This is a part of critical thinking.
Closing paragraph
Your closing paragraph should summarize what you discussed about your communicable or infectious disease. This should only be a few sentences.
Reference Page
The word Reference should be centered on the page. Arrange each of your references in ABC order. Remember that you need at least 3 scholarly references. Use APA with all your citation. Also remember to cite each article in the body (discussion) of your paper.
Remember to use the grading rubric as your guide as your write the discussion. The grading rubric will be used to evaluate the points you earn with this assignment.

Project # 2: (Paper) Assignment Purpose
This learning assessment activity serves to support learners’:
Examination of the relationship between vulnerability/social determinants of health and health disparities
Identification of population risk and protective factors relative to health outcomes
Prioritization of interventions for targeted population’s health based on assessment data
Exploration of the professional nurses’ role as population health advocate
Step 1: Select a Vulnerable or Special Population
Select a vulnerable or special population based on the ten (10) topics outlined within the lesson (see suggested resources in the table below).
Address each of the six (6) content areas listed below in your paper. Your paper should be in paragraph form rather than answering the outlined topics.
Identify the vulnerable or special population selected and your state.
Describe the factors contributing to the vulnerability of the selected population. Discuss the vulnerable population using the epi triad-host, agent, and environment
Describe the significance and economic impact of the population in relation to community and population health. Cite statistics to support the significance and economic impact. Remember to cite according to APA Guidelines
Identify demographic and epidemiological data relevant to your population.
Describe the characteristics in terms of demographics and epidemiological data (e.g., age, sex, marital status, economic status, employment status, race, ethnicity, household composition, health insurance coverage, access to health care, education levels, health conditions, etc.). Be sure to cite your sources appropriately.
Identify two (2) priority health concerns for this population and support your concerns with the literature. Cite your references in text to support your statements.
Discuss several evidence-based strategies nurses can implement that target the priority health concerns and the role of the nurse in improving the health outcomes for the population. You can implement using the levels of prevention-primary, secondary, and tertiary. Be thorough in your discussion.
Step 2: Research Your Population
You will have a minimum of five (5) references for this project including your textbook, a minimum of one scholarly journal article, and minimum of one web-based source. You may use the assigned reading article as one of your required journal article references.
Step 3: Prepare Your Paper
As you prepare your paper, use concepts and terms that have learned in the course. You might consider approaching the vulnerable population with host, agent, and environment. Also, look at prevalence, incidence, risk factors. Using the concepts that you have learned will add a dimension of critical thinking and synthesis to your paper.
Prepare a 3-5 page paper using a word processor such as Microsoft Word or PDF (includes title page to include the topic and reference page according to APA Guidelines).
Use APA Style to format your paper including a cover page, running head, and reference page. (These are included in the total page count.) Your entire document will need to be double-spaced to allow for embedded comments within your writing. Include an introductory and summarizing paragraph with your paper.
Remember to cite your references throughout the body of your writing to show where they influenced your writing and to support your conclusions. Note that direct quotations should be used sparingly and the paper should be written in your own words. Review your APA manual for formatting in-text and parenthetical citations and references.
