Assignment Directions
Step 1: Identify a client
Last week we discussed that, given the current causes of morbidity and mortality in the United States, the challenge for health care providers is to support clients to make healthy behavior changes. This week you will identify a client with whom you will conduct a case study on behavior change. Your client can be a colleague, friend, family member (no children without faculty permission), or patient with a health promotion issue (ie., smoking, obesity, lack of exercise, medication compliance, vaping, alcohol use, stress, work addiction). Keep in mind that the focus is on health promotion, not on an actual disease process.
Step 2: Select a Theoretical Model/Behavior Change Theory
Choose a theoretical model or behavior change theory and apply it as you work with the client to facilitate behavior change. This work will extend over the course of the semester. Most students find it helpful to meet with the client weekly and proceed step-by-step as suggested in the chosen model/theory.
In Week 2 you will complete Step 1 and Step 2. You will create 1-2 PowerPoint slides and a reference slide. You will identify the client and health promotion problem. You will discuss the selected theoretical model/theory and how it will be used to facilitate behavior change in your client.
Step 3: Progress Report–Week 7
In Week 7, you will submit a Case Study progress report. The progress report will highlight your plan of action, based on your selected theory and will include clear interventions that outline goals, time frames, and tools or constructs from the theory. The progress report will include:
Short and long term goals
Weekly intervention schedule
Time frames
Tools or constructs from theory
Step 4: Behavioral Change Case Study Paper
Write a paper of no more than six pages that includes the following information:
A brief overview of the theory/model that you used including strengths and weaknesses
The main concepts of the theory as well as an overview of past research using the theory
A brief history of the client’s problem
An outline of your Plan of Action
Describe how you implemented the theory; include details about how you applied the concepts and theory to your client’s behavior change
The outcomes of your case study
SUBMISSION
3-6 pages, double spaced (Note: your reference and cover pages do not count towards the 6 page limit)
APA formatting and citations
Your final paper must be submitted to your instructor by Week 13.
In Week 13, you will submit the final Behavioral Change Case Study Paper (regardless of which week you will present your Case Study in live session).
Rubric
Meets Expectations–90-100
Assignment includes clear and comprehensive review of the selected theory/model. Behavior change problem is clearly identified. All elements of the assignment are addressed substantially. References in APA format. Assignment submitted on time.
Basic–83-89
Assignment includes the components of the assignment, but may lack context or not be comprehensive. References do not meet APA standards. Assignment not submitted on time.
Unsatisfactory–82 and below
Assignment does not present correct and relevant information. References do not meet APA formatting guidelines. Assignment not submitted on time.
