Unit 5N is a general medical–surgical unit. On the night of the incident, staffing is considered adequate for the census and patient acuity. Mr. Jones, admitted for pneumonia, unexpectedly suffers a massive MI. The nursing staff responds to Mr. Jones and begins CPR while waiting for the “code team” to arrive. While the nursing staff is administering CPR to Mr. Jones, Mrs. Smith, an elderly stroke victim suddenly needs to go to the bathroom. She has been instructed to wait for assistance before getting out of bed and she is mentally competent to understand these instructions. When her call light is not immediately answered she decides she cannot wait and attempts to get to the bathroom (in this case a bedside commode) on her own. During her attempt to get out of bed, she falls and breaks her hip.
