Module 5 Assignment
Unit Plan* – Please detail out your scope and sequence of presentation of your materials. Some of you will just be doing a one-session program, so this is not as applicable where others are executing over a 6-12 week period. Typically unit plans are one page in length, think about your Course Guide as a unit plan.
* If you do not have a traditional education component to your health program, please write a protocol for events that need to happen for your program. For example, if you are working on policy changes at the worksite then there are certain hoops to jump through to make that happen.
Lesson Plan Sample** – please submit either your single session lesson, that outlines a timeline, content material, activities, and equipment needed. If you have a multi-session program, you only need to turn in one day/session.
**If you do not have a traditional health education strategy then please detail out the strategies you plan to use, referring to chapter 8 in the book.
Supporting Materials – Please also include any supporting materials such as PPT, video links, text messaging prompts, or anything that would be applicable to your programming.
