1. Intervention: (suggested length ½ page).
a. What problem (mental health, behavioral, social etc.) is addressed by the practice being evaluated? (What is the client being treated / intervened for?)
b. What intervention model is used by the practitioner whose practice will be evaluated? (What is the method of treatment? CBT? SU counseling? ABA therapy? etc.)
c. Time period of the of the intervention being evaluated (treatment plan: 3-month? 6-month? etc.)
