Using the Red Tool Kit (from the Important Documents) analyze more thoroughly Mr. Khan’s case. Begin with Step 2. Work through the parts in this step to determine how to work through the concerns and support making the decision of best placement and support needs. Use the Red Tool Kit pages 23 – 46 as your guide. However, you will need to do further research to understand his issues surrounding his chronic condition. Determine a plan for Mr. Khan that will allow him to keep active and yet receive the assistance he needs to remain out of the hospital. Consider and discuss the Medicare Readmission Reduction Program and the affect on Mr. Khan. How can telehealth and wearable technology help?
