Structure of the Integrated Report
Cover Page:
One single customary cover page: Plain APA style, no colors, no fancy designs.

Table of Contents is similar to the example in page 597 of the textbook.
Please note: Page numbers should be assigned until you have finished the entire document and is ready. This means no further changes, additions, deletions or page re-arrangements, because pages and its page numbers will move and your table of contents would have to be updated/checked every time you make adjustments.
List of Tables and Figures:
Similar to the example on page 598 of the textbook. Make sure tables (if you have any) follow a sequential order thought the entire text. Abstract:
One single abstract for the entire integrated document. No more than 150 words
Statement of the Research Problem:
Paste the meat of the final draft of this assignment (exclude: cover, abstract, references, appendices)
Methodology:
Paste the meat of the final draft of this assignment (exclude: cover, abstract, title of the study, references, appendices)
References:
Compile in a single list all of the references you used for the two assignments Statement of the Research problem, and the Methodology. In alphabetical order, eliminate repetitive references (the ones you may have used for two assignments)
Appendices:
Be careful with this.
Appendix A from Statement of the Research Problem will continue to be Appendix A for the integrated document. Appendix B the same as above That is, all appendices from Statement of Research Problem maintain their Letters ( A, B , C, etc.)
The problem comes with the Appendices of Methodology: Appendix A of Methodology no longer can be ‘A’ but the letter following the last Appendix letter of the Statement of the Problem. Therefore, wherever you mention any Appendix across the document, you need to assign a new letter to each Appendix.
In other words, Appendices of Methodology, they will lose their former letter and acquire the one that follows in the sequence.
Use your common sense.
NOTE: Appendices do not follow APA rules, because they have to be managed to fit sometimes the less space possible or the format of surveys requires specific fonts, spaces etc. Then, do not use double spaced rows in Appendices!

Remember (with exception of Appendices) APA format should be maintained throughout the document, one inch margins, double spaced, etc.
