information, if any, would you like to know that was not included in the case?
From
the perspective of the information which has been offered in the case scenario,
there is a need to provide more information on the medication which the patient
has been using. Also, there is a need to have more information on the history
of family medical reports such as anxiety or any other mental disorders.
Besides, the patient should be able to give information on when the condition
began.

Which
psychiatric symptoms are a treatment priority for this case?
The
psychiatric symptoms which are treatable for the case are such as the issue of
panicking when the patient faces some experiences. Besides, there is a need to
treat symptoms such as lacking social interaction with people around the
patient. There is also a need to treat symptoms such as overthink and racing
thoughts.

What
are the non-pharmacologic issues in this case (problems/complaints that cannot
be addressed by medication)?
The
non-pharmacologic issues that cannot be addressed by medication use are having
thoughts that are racing. Also, there is the issue of having an unpleasant
experience that the use of medication may not address. Besides, the increased
heart rate may not be addressed by the use of medication. Also, decreased
socialization and exercise, and loss of interest in sex.

one medication that would be appropriate for this case. Include the name and
starting dose.
One
medication that is recommended is that of Prozac, and the starting dose for the
patent, in this case, is 20mg per day, and the patient should have a continued
dosage of the same medication at 90mg on a weekly basis (Petersen, 2019). By
taking such a medication, it will help in reducing panic disorder and the issue
of reducing anxiety when making social interactions.

your clinical decision-making. What is your rationale for choosing this
medication? Also, include the mechanism of action for this medication choice
and the neurotransmitters and areas of the brain on which the medication is
proposed to act on.
Upon
examining the patient against all the information provided, the clinical
decision made is that the patient is suffering from severe anxiety disorder
based on the symptoms found in the patient. The patient has a lot of worrying,
which makes the heart rates have a high rate of pacing. The patient is also
experiencing social disorders, which increase the rate of anxiety disorder. The
rationale for medication to be prescribed to the patient is based on the
acuteness of the disorder as the patient will need a medication together with
psychotherapy which will aid in reducing the symptoms where the medication will
not work.

laboratory testing/monitoring is needed for safely prescribing this medication?
The
laboratory testing needed to prescribe the medication in a safe manner is that
of the blood pressure. The need to take the test is to ensure the patient has a
heart rate that is not related to hypertension or diabetes.

there any contraindications or safety issues associated with this medication?
The
medication has had various contradictions, such as the drugs reacting to
patients who have been suffering from diseases such as diabetes and
hypertension. As such, the patient who takes the drug may be involved in
excessive bleeding. There is also a contradiction on the issue of increasing
the suicidal thoughts for the patient.

The
patient will be recommended to undergo cognitive behavior therapy as it will
help change the behavior, such as social interaction with the husband. In this
case, the recommendation will not be limited to psychotherapy only but will
extend to holistic therapy and complementary treatment (Oswald, Francis &
Dhakal, 2019). The holistic and
complementary therapies will help ensure that the patient can relax the heart
rate by encouraging the use of body and mind techniques.

are the safety concerns, if any, associated with this case? How will you
address safety?
Based on the case, the safety concerns
are such as increased chances of developing depression. Also, there is a
concern for substance abuse where the patient will want to have fast healing,
thus leading to the overuse of drugs (Blumenfeld et al., 2019). The safety concerns will be addressed by
ensuring that the patient is treated using the correct procedures and
medication for a given period to avoid safety issues.

When
would you follow up with this patient?
The
patient is best followed up in a period of two weeks after having taken the
right medication and after undergoing the correct therapies.
