CASE STUDY

Alan Sharpman, director of the stroke recovery unit at Barrymore Rehabilitation Center (BRC), closed his office door and sighed. Most of the time, he liked his work. Helping someone lead as normal a life as possible after a stroke was gratifying. Most of the time, the organizational culture worked well with an emphasis on independent individuals working together as a team. The team members were smart and worked independently without needing Alan’s assistance; this allowed Alan to use a laissez-faire style of leadership which was comfortable for him.
The BRC patients exerted great effort to recover and always seemed to express appreciation for Alan’s and his staff’s help in the rehabilitation process.
The staff at BRC functioned as interprofessional teams that focused on the individual therapy of each patient. Barr were divided into interprofessional teams that focused on each individual’s therapy. A nurse, a speech pathologist, an audiologist, a physical therapist (PT), and an occupational therapist (OT) would come together to plan, implement, and evaluate the specific rehabilitation treatment for a patient. The team members, who came from about the same hierarchical level, worked together to help the patient recover. “Restore the body, empower the spirit” is a philosophy to which the team members subscribed. That is, they subscribed to it most of the time.
Alan sighed again and said to himself, “Today is not one of those times.”
The audiologist Sam Turner; a nurse, Susan Baker; and the speech pathologist, Sara Ayers all had approached him to discuss the behavior of two of their interdisciplinary team members: John Sanducci who was a PT and Tom Mallory who was an OT.
John was one of the first PTs hired at BRC thirty years ago; he is well respected and knows the unit’s procedures better than anyone. He is often sought by other PTs for advice and is extremely conscientious about staying up-to-date on treatments and protocols. He is often heard saying to the staff how much he likes helping patients, “restore the body and empower the spirit.”
Tom, a 26-year-old, is the newest employee at BRC and is a recent graduate from the local university’s OT program. BRC is Tom’s first full-time employment. Tom has bundles of energy and is full of ideas; he is always ready to take on the next task. He gets so excited but it is difficult to get him to stop talking so others can talk.
Sara had explained that although each of these guys (John and Tom) were great to work with individually, they cannot seem to get along with each other.
Sam told Alan that when Tom and John work on the same team they cannot agree on anything. It seems if Tom, recommends anything, John opposes it. And it is difficult for John to get a word in so when he does get a chance to say anything, he usually just shouts at Tom.
Susan added, “we told them to stop fighting and get back to discussing the patient,” and they both just refused to participate. Susan lamented that, “We are just so frustrated with the time wasted. We are not accomplishing as much as we could.”
