Business Operations Manager Candidate Trial Activity
You are the new Business Operations Manager at a Success Academy Elementary School (grades K-4). In your role, you are responsible for managing scholar health form collection. Create a project plan to ensure you collect health forms for 100% of scholars. Your project plan should include guiding principles or a narration of your overall strategy, an action plan with clear ownership and benchmarks, and a communications plan with talking points.

● Objective: Collect complete health records for 100% of scholars.
● You have 30 days to achieve this outcome.
● For a scholar’s health record to be considered complete, your school must collect the following
from each scholar:
○ Scholar health examination form
■ Also known as an “annual physical”
○ Scholar immunization records
○ Scholar Medication Administration Form (MAF) and their required medication
■ MAFs are required for scholars who need to take over-the-counter or prescription medications at school; scholars cannot use or be given medication at school without an updated MAF.
● When you begin managing this project, your school has the following completion rates for each form:
○ Scholar physicals: 65% Complete
○ Scholar immunization records: 90% Complete
○ Scholar MAFs (for scholars who take medication at school): 62% Complete
■ Scholar medication: 43% Complete
● 350 scholars attend your school
● Your operations team is comprised of the following individuals:
○ Senior Ops Associate: Mackenzie (3 years of experience)
○ Ops Associate: Jordan (1 year of experience)
○ Ops Associate: Alexandra (2 months of experience)
● Instructional staff at your school:
○ 30 teachers
○ 2 Assistant Principals
○ 1 Principal
