Pages 1-4 These pages explains the assignment and gives examples of what is expected.
Pages 4-5 This is the case study we are to use for the assignment. The student is named Frieda.
Pages 6-7 This is the rubric being used to grade the assignment.
Pages 8-9 This is the chart we are to use to complete the Individual Adaptation Plan.
The assignment has two parts:
1. A report on the Present Level of Performance for Frieda (page 4-5) and 3 realistic goals for her. Each goal has to have 3 objectives. *There are great examples in the attachment!*

2. Use the bottom of page 1 to help you fill out the Individualized Plan Chart at the bottom of the attachment.
It looks overwhelming at first but it’s not as bad as it seems. Please reach out if I need to clarify anything. I know there are a variety of writers, coming from all over the world, and phrases we use in the United States can be confusing or odd. Trust me, I think so too.
