RESPOND TO EACH DISCUSSION BOARD
1. The goals for dissemination of evidence-based practice include increasing the reach of evidence, increase an individual motivation and ability to use and apply evidence. One of the internal methods for the dissemination of my evidence-based change proposal would be medical director. An external method is use of a professional nursing organization. A good technique must be used to identify the individual or organization that needs to be included in the program. To identify the suitable method, the following process is used. First identify the internal and external stakeholders, then assess the nature of each stakeholder’s influence and importance in the organization. Follow this by construction of a matrix that identifies stakeholder influence and importance. Finally monitor and manage stakeholder relationships. It is important to report the results to both groups. This will enable the outcome of the research of the project to be perfect and hence more informative. The communication strategies for each group will change. This is where a good communications strategy is helpful. An internal method is approached different with the external methods. In the internal method, you cross all the information whereas in the external method one need to have the limit of the information you give out.
2. Effective wound care treatment requires redesigning systems to reduce risk factors and to optimize evidence-based processes of care. To evaluate an evidenced-based practice project, it is important to be able to determine the effectiveness of your change. Measurement is essential to monitoring success and helps guide your team towards your specific intervention goal. The easiest way to determine the effectiveness of the change would be to compare the wound healing rates from before the intervention to after the intervention.Every patient who has a wound should be seen in clinic weekly to measure the wound, debride the wound if necessary, monitor for infection and change treatment if necessary. One way that I will be able to evaluate whether my project made a difference in practice is the increase wound healing times. When an increase in wound healing occurs then health care professionals will realize that taken certain measures/interventions can greatly affect the way in which we increase wound healing times.
3. One way in which I will be able to evaluate if my project has made a difference in practice is through gathering data and evaluating to compare the population utilized for the study versus those who are not involved – looking for alterations in coagulopathy and an increase/decrease in adverse complications (particularly those to do with ICU delirium and/or excessive clotting, such as MI, CVA, PE, etc.). Observations and data collection associated with possible bleeding complications due to excessive anticoagulation would also need to be analyzed, to ensure that while we are attempting to aid this particular populations and prevent avoidable complications which may be fatal, we are not also causing complications in which the benefits of the change proposal do not outweigh the risks
4. Evaluation of an evidence-based practice project involves determining the effectiveness of its change. One way that enables an assessment of whether the patient education intervention made a difference in practice is through a control group. The PICOT question facilitates the evaluation by mandating a comparison: In general patient population outpatient and inpatient, what is the effect of the educational process with pre-stress test compared to normal care to improve patient safety and quality of patient care within three months? A randomized parallel-group design will be the methodology used in the study, as highlighted in the evaluation plan, to answer the PICOT question. Half of the patients enrolled will be randomly assigned to receive a patient education pre-test, and the remaining will not receive specialized education. Data from two follow-ups, at one month and three months, post-test, will be analyzed to determine whether there will be any significant change. The magnitude of the change or lack thereof in the intervention group compared with the control group will provide a clear indication of the effectiveness of the patient education project.
