This case study introduces you to Margaret McNolan, an 80-year-old woman living in a small town of 800
people near a medium sized regional city in southeast Queensland. “Marg” was previously married and has
two children, Matt (58yo) and Helen (55yo). Marg was divorced in her mid-30s but remarried Tony in her late
60s; Tony died only three years ago, although his own family remains involved in Marg’s life.
Marg’s life has been hard. She has always lived in her town, married a local man and raised her two children
there. After her first husband left suddenly, Marg had to work at the local dairy factory to make ends meet.
Her kids moved away once they left school, although Helen now lives in the nearby city. Marg is something of a
local character in the town – she’s a straight-talking, no-nonsense woman whose strident opinions on most
matters are both admired and feared!
Diagnosis and prognosis
After Tony’s death, Marg finally went to their GP about her shortness of breath and chronic cough, which she’d
put down to the exhausting demands of caregiving and, she reluctantly admitted, her decades-old, heavy
smoking habit. After investigation, it was established that Marg had chronic obstructive airways disease
(COPD) with a post-bronchodilator FEV1/FVC <0.7 In the three years since, Marg has become more and more frail, with marked weight loss and increased breathlessness even at rest, which is severe at times. She uses intranasal oxygen via a concentrator at home. Helen has been asking to be appointed as her mother’s Enduring Power of Attorney and Marg has dismissed her requests, suggesting that Tony’s son Ken, a lawyer, should do it. This remains unresolved. Clinical presentation Marg has been admitted to the medical ward in the regional hospital following a sudden increase in her shortness of breath. She is alone, although her daughter is on her way. On physical examination, you observe that Marg is obviously short of breath, with an audible wheeze, and requires great effort to breathe in and out. Her respiratory rate is 26 breaths/min and her pulse rate is 110. Marg’s temperature is 38.0°C and her cough is intermittent but productive of dirty yellow phlegm. She has dusky discolouration of the nail beds and lips, and says she fees ‘utterly exhausted’. She has a mild tremor in the hands. Marg reports being ‘bunged up for days’ (ie: constipated). She seems very agitated and anxious. Urban Partnerships to Address Health Literacy in High Need Populations

Low health literacy disproportionately affects racial and ethnic minority communities and lower-income socioeconomic groups. To address this critical determinant of health inequity, two nonprofit organizations, Repairers of the Breach, a day shelter for individuals experiencing homelessness, and Bread of Healing a neighborhood-based clinic serving individuals with low incomes, partnered with researchers at Marquette University to implement and evaluate an evidence-supported health literacy program. The partnership delivered the curriculum in seven one-hour sessions over seven weeks. The program attendees were predominantly African American men and women from 19-73 years old. Most participants had formal education ranging from elementary school to some college. Forty individuals attended at least one class and 14 attendees completed 4 or more classes. Program completers demonstrated gains in confidence and topic knowledge. Most interviewees reported a personal/family need for the program, acceptability of the group format, and the ability to learn the skills they needed for self-care. The project used a successful collaboration between community-based organizations serving vulnerable populations and an urban academic institution to demonstrate the necessity, feasibility, acceptability, and effectiveness of formal health literacy education in adults with low incomes or who are experiencing homelessness. Urban and metropolitan serving institutions can work in partnership with community to address low health literacy in high need populations.

The body systems that control the growth and maintenance of the skeleton can be disrupted in different ways that result in a variety of bone diseases and disorders. These include problems that can occur at or before birth, such as genetic abnormalities and developmental defects, as well as diseases such as osteoporosis and Paget’s disease of bone that damage the skeleton later in life. In addition to conditions that affect bone directly, there are many other disorders that indirectly affect bone by interfering with mineral metabolism.

In this discussion we will focus on disorders of some of the groups of bones that compose the skeleton, and examine skeletal problems at the organ and system level as opposed to the tissue level treated in the previous discussion. we will review some of the more common diseases, disorders, and conditions that both directly and indirectly affect bone.
Lateral forces, twisting stresses, and powerful impacts may cause the bone to break, or fracture. There are two types of fractures: closed and open. A closed fracture occurs when the skin is not broken. An open fracture is when the skin has been broken and involves an open wound. This type of fracture is more serious because of the increased risk of infection and shock caused by blood loss and damaged tissue.
Keeping the broken body part stationary — immobilizing it — will stabilize the injured area and prevent the bones from shifting until further treatment is available. Splints are used to immobilize an area. They can be made from cardboard, newspapers, sticks, or any other rigid material.
When a cast is needed, the doctor first checks that the bones are in correct alignment to promote proper healing. A cast is then placed over the fractured area to immobilize it, allowing the bones to grow back and heal in the desired configuration.
Although fractures are an unfortunate occurrence, most of us can take precautions to avoid them. There are certain diseases, however, that make some people more susceptible to fractures.
Diseases of the bone, including osteoporosis, rickets, and osteoarthritis can cause bone deterioration or fragility. This can increase the likelihood or severity of a bone fracture.
For this basic study of diseases or disorders of skeletal system, in a paragraph or two briefly discuss ONE disease or disorder.

The integumentary system is susceptible to a variety of diseases, disorders, and injuries. These range from annoying but relatively benign bacterial or fungal infections that are categorized as disorders, to skin cancer and severe burns, which can be fatal.
