Care Coordination Plan Assignment

The purpose of this assignment is to identify strategies and interventional steps for building cultural awareness in varied health care contexts.
For this assignment, you will:
1. Identify a health care need in your area (e.g., access to health care, aftercare resources for patients in need, lack of support for outpatient mental health clients).
2. Explore local resources that might be utilized to address this need in an interdisciplinary manner (e.g., Church groups, Public Health Departments, etc.)
3. Create a Care Coordination Plan for a cultural group based on your research of resources. The plan should contain:
a. a description and analysis of the health care need (5 points)
b. a review of at least four (4) community resources (5 points)
c. a listing of five (5) interdisciplinary steps designed to address the need (5 points)
