Step #1, Create a blame-free environment.
As a hospital manager, I would ensure that employees are able and feel comfortable reporting errors and near-misses without fear of punishment or retribution. Medication errors account for a high volume of patient harm in the healthcare field. By creating a blame-free environment, we can learn from our mistakes and learn how to prevent them from happening in the future.
Step #2, Involve employees in decision-making.
As a hospital manager, I would enable employees to be able to participate in certain decision-making activities. When employees are encouraged to offer ideas, and see that their contributions are taken seriously, they will be more satisfied and productive. Engaging employees in conversation with management and each other about safety and health can lead to better communication, reduced errors, and improved morale.
Step #3, Provide continuous learning opportunities
Both employees and management should be able to constantly learn and grow on the job. This can both improve confidence in the job and also offer advancement in the workplace. As a hospital manager, I would offer additional training for interested employees, and consider offering incentives or recognition for those that complete additional training and education.
2. As the hospital CEO, tough decisions that impact team members directly are commonly addressed. Some of these tough decisions may involve the cutting of funds or the addition of services. The main goal of my job is to provide safe care for our patient population and maintain employee engagement. When the first goal of the administration is safety, it becomes easier to address the concerns at hand.

Step #1I would start by ensuring the orientation outlines the safety goals of the hospital. If all team members are aware of the goals and visions of the hospital, along with ways to ensure they are being implemented. Clear communication expectations help to ensure concise communication from all administration. The administration must also be present during onboarding to ensure that the message being relayed to all team members is consistent.
Step #2: I want to ensure all team members are given real hands-on experiences and related information during onboarding. When learning in a classroom environment, many people are noted to pass. When providing case scenarios on real-life examples that have been experienced, the team members can translate learned materials into practice. Safety events help to provide a self-reflection of knowledge gaps if present.
Step #3, The third step I would instill into the hospital practice is an open-door policy. Open-door policies allow team members to freely reach out to leadership to ask common and complex questions. The comfort and rapport created with team members help to solidify a safe practice culture. In addition, the culture of learning from errors is necessary to ensure that team members can communicate any safety concerns that may arise. Most recently, one of the neighboring hospitals just experienced a team member abusing narcotics and was noted on-site taking care of patients. Communicating concerns from team members to the leader created the ability for a thorough evaluation of the team member and the eventual removal of a safety concern. Examples such as these allow the team members to feel safe that our administration team will act appropriately and investigate their concerns.
