This module, you will be pulling together all you have learned throughout the course as you create your own program and evaluation proposal for a program to address the community problem you identified in Module 2. Your program proposal should follow the below outline. When writing up your proposal, you should use headings and subheadings throughout your proposal that align with this outline.
1. Executive Summary a. Title of the program b. Name of organization c. Program mission and vision d. Program goals e. Overall description of key program activities f. Overview of key stakeholders
2. Background/supporting information a. Background of the problem being addressed (This section should be 1-2 pages with a minimum of 2 scholarly resources) b. Context of the program including community characteristics and relevant information on impact of this issue within the local community c. Discussion of the need for this program include gaps that exists is current services being offered in the community
3. Goals, action steps, and outcomes a. A clear statement of each goal b. Under each goal should be a detailed discussion of the action steps to achieve the goal c. Under each goal state the indented outcome in measurable terms. d. Conclude this section with your updated logic model
4. Funding a. Overview of the ways in which you believe this program can best be funded
5. Evaluation plan (to be conducted once program has been in place to determine the effectiveness of the program) a. Type of data to be collected (quantitative, qualitive, or mixed-methods) with a discussion of why this is the best fit for your program b. Sample – who will you collect the data from c. Data collection tool(s) – which instrument(s) will you use and why d. Stakeholders who will participate in the evaluation e. Ethical considerations.
This paper should be 5-7 pages double spaced and written in proper APA format. In addition to the minimum page count (i.e. 4 pages and 1 line minimum), you should also include a cover page and a reference page in APA format.
Additionally use a minimum of 5 additional sources outside of your textbook to support your points.
