Purpose In this assignment, you are required to demonstrate your skills in conducting a comprehensive
mental health assessment of a consumer that is receiving services from an acute mental
health setting/facility or who is presenting with issues of an acute nature.
Description
Suitable settings/facilities include inpatient, community, emergency departments or other
acute settings. Please note, all information is to be de-identified and you must use a
pseudonym for the person. If you are unable to identify a client within any of the above
settings/facilities, undertake this assignment with a consumer you are working with that has
identified mental health concerns.

This assessment requires you to provide the following:

Part 1: A succinct yet comprehensive mental health assessment utilising a biopsychosocial
approach. This assessment must include a comprehensive history, and assessment of
physical health, psychosocial health, vulnerabilities and strengths.
There must be evidence of a collaborative approach that is consumer focused, and recovery
orientated and describes engagement and the therapeutic relationship. It must clearly
articulate the consumers’ goals, wants and views of their current situation and future plans.

Part 2: Provide a formulation of the problems (arising from the mental health assessment)
including: a summary of symptoms of mental illness, the consumer’s perspective and context.
NOTE: This formulation will be required to be submitted as part of Written Assignment 2
(Assessment 3).

While this is a clinically based assignment, your assignment must be supported with
contemporary high quality references (avoid a reliance on health policy documents as your
sole source) and demonstrate your capacity for sound clinical reasoning.

NOTE: This assignment will be the basis for your collaborative care plan that will be
developed for Written Assignment 2 (Assessment 3).

If you are not currently working in a clinical area and are unable to access or identify a suitable client to engage with for the purpose of this assignment then you may choose to use the case study presented in the video below. Please ensure the case study is referenced as part of your assessment.
