Part I. Define your topic of concern and conduct a relevant needs assessment.
Description: Define the topic you are addressing. How are you conceptualizing the issue? Consider the following questions– How many people does this impact nationally or locally? Why is this issue important? How does this social problem impact people?
Needs Assessment: Apply the four types of needs assessment as appropriate. Use professional sources to gather information when possible. If you are not able to capture enough information with current resources, state how you would collect needs assessment data.
Notes on this section: This section will require at least 4-6 references. Be sure that all references are professional and appropriate (peer review, government documents, etc). This section is setting the stage for your program so the background and needs assessment data should clearly state why this is important.
Part II. Rationale for the Proposed Program
Program Hypothesis. Provide a rationale for why the program you are proposing is needed and appropriate for your issue. Clearly state the program hypothesis.
Logic Model. Present a clearly articulated logic model for your program. The logic model should identify specific inputs and expected outcomes.
Part III. Program Description
Program Overview. Clearly describe the type of program you are proposing. The program overview should align with Sections I and II of the proposal. The program overview should also address the context or scope of the program.
Specific Activities. Describe the specific activities that will occur in your program. Who will implement the activities? Where will they happen? How many people will be served by your program?
Part IV. Evaluation Plan and Budget
Evaluation Plan. Describe how you propose to gather information related to your program and how that information will both inform improvements and also how information can demonstrate the effectiveness of the program (process and outcome).
Budget. Identify the costs associated with your program. Be sure to include space, technology, staffing, etc. Include a budget spreadsheet and a narrative portion that explains the costs.
