Describe the resources that is required to achieve the goals For our networking project, we propose supporting chronic management of congestive heart failure through the use of remote patient monitoring. This topic was chosen because it represents a capability that has a promising future in advancing the quality and delivery of healthcare by utilizing telecommunications to promote continuity of care and improve disease outcomes while reducing the high costs associated with chronic disease management. Remote Patient Monitoring (RPM) covers a broad range of opportunities in healthcare. The use of RPM for the chronic management of congestive heart failure (CHF) is a well-documented research topic that will be beneficial for the scope of this project)
