You are expected to perform an office physical ergonomic evaluationand provide documentation. Essentially you are performing a workstation JHA. You are to identify and document an office workstation setting (possibly your own or someone elses)and evaluate it for unsafe physical conditions.
You are(1) to completethe form provided(W5 A1),
(2)take real picturesof a worker in the office space (take different angles)
(3) using the form provided as a guide place the real picturesinto a PPT presentationwith arrows depicting at risk hazards and appropriate controls if present or needed
(4)upload both the PPT slide (no more than 2-4slides) AND the completed form into Blackboard.
In your PPT,you are to identify any office physical ergonomic hazards,controlsor potential controls,you can put into place to address deficiencies. In the notes section of the PPT slide you are to place an explanation of the hazards or controls you identified. NOTE: If your DO NOT complete the notes section you will lose 20% of the total points of the assignment.
