Case Study – Maximum 5000 words Students should choose one of the following two Case Examples and in each case discuss the questions identified. Students should write up to 5,000 words. All assignments are to be handed in by the deadline of: 12th May at 14.00 Case Study One Sally Appleton Sally and Joan Appleton live in council housing in Anlaby. Sally is 53 years old and is the only child of Joan who is now 82. Joan’s husband Clifford (Sally’s father) died in 2007. Joan had worked in schools and factory canteens when she was a younger woman but left work when Sally was born. Joan receives a small monthly payment from her husbands’ private pension scheme and also draws her own state pension. Sally is described as having a moderate learning disability with some additional difficulties, for example, with anxiety and lack of confidence. Sally is currently unemployed and receives disability benefits. Sally receives support from mum Joan with many aspects of daily living and personal care. Essentially, Joan does all the cooking, shopping cleaning and helps Sally getting into the bath. Joan helps Sally with her hair every morning and reminds Sally to use her deodorant. Sally is socially skilled in the contexts that she is familiar with, and with people who are honest and clear with her. Sally struggles with new people and new situations and becomes very anxious. Sally attends CASE Training Services for 3 days a week, and also attends a council run day service in Cottingham, for one day a week. She is a very popular member of all groups that she attends and much enjoys socialising. Sally has much affection for her friend Peter, who she has known for a long time, though she sees him perhaps only once or twice a month at CASE. Sally sometimes refers to Peter as her ‘boyfriend’ though Joan sees him as little more than an acquaintance. Peter does not visit Sally at home. Joan is very happy to look after Sally and is proud that they do not have to rely on any social services home help in their daily lives – when she is asked about whether she would like Sally to move into a care home, she is clear that this will only be ‘over my dead body.��� Joan suffers from chronic high blood pressure and deep vein thrombosis (blood clots in the veins of her legs) though her symptoms are reasonably well controlled with a range of medication and other treatments. She has regular check up’s with her GP and the ‘warfarin nurse’ at her GP’s clinic. Tracey and Tom are Sally’s neighbours. They have been married for 8 years. Tracey is in her late 30’s and has known Sally and Joan all of her life. Tracey sees Sally and Joan almost every day; she pops in for tea and a chat and either Tom or Tracey will drive Joan to the supermarket once a week. Tracey and Tom do not have any children. Tracey works part-time and Tom full time. Sadly, Joan Appleton is admitted to Hull Royal Infirmary having suffered a serious stroke at am on January 27th. Tracey found Joan semi – conscious on the settee in the lounge when she popped in to check whether Joan needed anything from town. Sally is at CASE and due to return home at pm. Tracey phoned social services at am and explained the situation and what had probably happened. She made it clear that she could look after Sally that evening and the next day, but if Joan was kept in hospital longer, the council would have to ‘sort out a home help or a carer’ for Sally. The telephone message is passed on as an urgent referral to the adult social care team for a response. The team manager of this team picks up this referral at and allocates the work to a social work practitioner in the team.
