On Dec. 7, 2000, the Cincinnati Occupational Safety and Health Administration (OSHA) office heard through media and police reports that there were two deaths at a nursing home in Ohio. OSHA determined that the Food and Drug Administration (FDA) should take a lead role in performing an investigation.

Because the nursing home had many residents who had unhealthy respiratory systems, the nursing home routinely ordered and received tanks that contained pure oxygen. During one delivery, the supplier mistakenly delivered one tank of pure nitrogen in addition to the three tanks of pure oxygen that had been ordered. The nitrogen tank had both an oxygen and nitrogen label. An employee at the nursing home connected the nitrogen tank to the nursing home’s oxygen delivery system. This event caused two nursing home residents to die, and three additional nursing home residents were admitted to hospitals in critical condition. You are developmental psychologist working in the Peach county school system. Your job is to design programs to maximize the learning of children who are having problems in school. Select a specific theoretical orientation that you strongly “believe in” and design a program that fits this theory. By offering a sound CC, you are creating a focused problem areas (step 2 of inverted pyramid model, IPM) that directly translates to the treatment plan format, theoretical buckets (e.g., cognitive, and behavioral maladaptations, if using CBT), and an etiological understanding (e.g., maladaptive core beliefs) which are driving the maladaptive thinking and actions.

INSTRUCTIONS

To complete this Case Presentation: Case Conceptualization (CP-CC) Assignment, do the following:

1) Review the Case Presentation: Case Conceptualization (CP-CC) Template document. Follow the APA format as provided in the template. It is developed to help you stay aligned with assignment expectations based on the assignment instructions and grading rubric.

2) Review your textbook and other readings on Case Conceptualization and review theory-specific examples.
3) Write up the part 1 of the Inverted Triangle Model based on Case Presentation: Assessment and Diagnosis Assignment data.

4) Write up part 2 of the IPM. See p. 90 of Switzer and Rubin for guidance. This is the atheoretical grouping of part 1. Make sure to document in the assignment which of the 4 thematic grouping approaches was used for step 2.

5) Next write part 3 of the IPM. This where you apply your chosen counseling theory (psychodynamic, CBT, etc.). Use only 1 theory. As a counselor-in-training, you do not have a solid foundation for more than one theory as it takes years to master. As such, you would be operating unethically and potentially illegally, based on ACA code of ethics (2014) core professional value 5; C.2.b; C.2.f; and E.

5.b. You will use your chosen theory to determine the etiology, deeper theoretical inferences. From a CBT lens it is the core belief, from a psychodynamic the drives, instincts, unconscious factors that are driving the themes in step 3 of the model and ultimately the signs and symptoms observed in step 1 during the assessment and diagnosis process.

6) After completing the IPM, it is now time to write up the CC narrative. The CC narrative is where you apply your chosen theory to explain (not describe, as you did in the diagnostic impression) what is going on with the client. You use key theory-based concepts and terms to explain why a client is experiencing their signs, symptoms, problems.

7)You do not do a Christian integration in the CC narrative. Only use your secular counseling theory.
