Directions: Working with your group, research the population characteristics, health trends, and epidemiological data of your assigned area. Based on the data collected from the Windshield Survey/Community Assessment, evidence-based nursing practice, Healthy People 2020 as well as specific research conducted, identify a problem, issue, or topic that is relevant to the targeted community, which may include, but is not limited to:
population (eg. elderly, children, homeless, underserved)
health/illness (eg. heart disease, hypertension, diabetes, cancer)
socioeconomic (eg. smoking cessation, vaping, STDs, access to healthcare)
behavioral (eg. safety, crime, drug use)
other (eg. teen pregnancy, prenatal care, mental illness)
Develop and implement a program that addresses the issue/problem/topic. The program could be an educational presentation related to health or safety, a health fair focusing on several topics, an event created in collaboration with other disciplines, volunteering in one of our allocated community health agencies, or some other creative project. Following your project, a group paper will be written and submitted by one student in the group.
All programs must be approved by the instructor before implementation, and you must have approval to present at your selected location within the assigned zip code.
Include rationale based on evidence from Healthy People 2020, Florida Department of Health, Jacksonville Community Council, Inc. (JCCI), or other sources. In your document discuss the identified problem/issue/topic and reason for selecting it.
Describe the population characteristics, health trends, and epidemiological data of the specific zip code area. Address each of these 3 topics thoroughly in separate paragraphs of at least 3-5 sentences.
Provide evidence and/or data that this is an important problem/issue/topic which should be addressed.
Identify method of teaching used and teaching/learning strategies applied to address this problem/issue/topic (eg. health fair, educational program) and reason(s) for selecting this method. Include strategies and considerations to be used to plan an educational program or event.
Summarize program overview and method of implementation as well as location (eg. library, park, school, local business) where program was implemented. (It is the responsibility of the group to identify the location and obtain approval from location site and course instructor.)
Evaluate activity/project in the Conclusion, if not included in program overview. (Include group’s evaluation of project, number of people reached, community interest, and any changes you would make in the future for another project.)
