Description of incident:
A missing swab at obstetric (18 x 18) left in the patient as a near miss. It was noticed during the first count. Patient was booked for an elective Lower Segment Caesarean Section who also had fibroids. Staffs got to know during the team brief and the necessary equipment was provided based on the EBL.
I was double scrubbing with my mentor and so I saw the swab left in the patient to soak her bleeding.
During the first count my mentor noticed of a missing swab. I told her it’s in the patient but the surgeon denied it yet I could see a bit of the string in the swab showing.
The surgeon kept denying and even shouted at me because I was insisting that the swab was inside the patient. She finally found it when she was about to close the sheet and apologised.
Incident should be attached as an appendix.

You must apply no less than three theories/concepts discussed during class, from the textbook, and/or from your own research, and your understanding of the material. It is not okay to simply mention the theories/concepts. You must apply them to your victim(s) in an effort to explain how and why they wound up in the situation causing them to be a victim. Primary/direct and secondary/indirect victims will not be counted as one of the required theories/concepts.

First, your paragraphs are very short, almost like a newspaper editorial. You want your paragraphs to be around 2/3 page each.

Second, the first 5 paragraphs all seem to repeat the same information. How can you combine similar information using transitions and remove the rest to avoid repetition? A mini intro outline might help.

Third, be sure to introduce and discuss all quotes–don’t just leave them hanging there.
